A case in a 58-yr-old mestizo man is reported. He presented with a painful, swollen right knee which had developed following a minor trauma. Radiography revealed cartilage loss, a large erosion in the proximal end of the fibula and irregular bone density at the distal portion of the femur. Haemorrhagic fluid obtained at arthrocentesis grew P. brasiliensis. The patient was successfully treated with trimethoprim-sulphamethoxazole and amphotericin B (total dose 3 g).
