Cure of Paecilomyces endophthalmitis with multiple intravitreal injections of amphotericin B

A case is reported in a 43-yr-old farmer who developed fungal keratitis after injuring his right eye. Despite administration of topical natamycin and miconazole, the infection worsened and a therapeutic keratoplasty was performed. P. lilacinus was isolated from the keratoplasty specimen. Ten weeks later the patient presented with endophthalmitis and P. lilacinus was cultured from a vitreous specimen. Amphotericin B (Fungizone), miconazole and flucytosine were administered but the infection soon recurred. After a third recurrence of endophthalmitis, in an attempt to salvage the globe, eleven 10- micro g injections of amphotericin B were given intravitreally over 14 d. There was rapid decrease in pain and inflammation, and the patient suffered no further recurrence. Corrected visual acuity improved from 20/200 to 20/40.
