Current status of allergen immunotherapy. Shortened version of a World Health Organization/International Union of Immunological Societies Working Group report

Allergen immunotherapy (hyposensitisation) has been widely used for nearly 80 years to treat immediate hypersensitivity type reactions, now known to be primarily mediated by allergen-specific IgE antibodies. These reactions include rhinoconjunctivitis (most commonly provoked by pollens, and by Dermatophagoides pteronyssinus), asthma, and severe responses to insect stings. The availability of potent drugs for the management of these conditions has brought about changes in immunotherapy, but nevertheless the continuing occurrence of severe reactions to treatment, even deaths (albeit mostly in inexperienced hands, and only 26 such deaths reported over 30 years in the UK and 46 over 42 years in the USA), has led in various ways to a need for reappraisal. The present report reviews and/or makes recommendations on the following aspects: mechanisms of immunotherapy, clinical studies, standardisation, patient selection, recommended procedures, adverse reactions, precautions & prevention, management of reactions, and improvements in current procedures.
