A 12-year-old Lebanese boy developed the sudden onset of palpitations while playing. Physical examination was normal except for tachycardia. No murmurs were detected. The Casoni skin test was positive. Indirect haemagglutination showed a titre of 1:128. The white blood cell count was 9200/mmsuperscript 3 with 16% eosinophils. Chest radiographs showed a slightly enlarged heart with bulging of the left posterior heart border.
