A report of the 22 patients seen in a medical practice in the Irish Republic during 1992 in whom Lyme disease was diagnosed clinically and serologically. In 8 patients the disease appeared to have been contracted in the Irish Republic, whereas in the others exposure overseas had a major role (in these patients presenting features were diverse but the underlying symptom was waves of abnormal lethargy). Lyme borreliosis was serologically confirmed in patients returning form Nigeria, Angola, Kenya, Tanzania and Zambia. In most of these cases the patients had a history of tick-bite and 4 gave good descriptions of annular erythema. The patients responded well to oral doxycycline, or in some cases high-dose intravenous ceftriaxone. These cases are among the first series of serologically confirmed Lyme disease from tropical Africa, and it is recommended that doctors seeing patients returning from this region with significant lethargy should consider the possibility of imported Lyme disease.
