A case of haemophagocytosis is reported in a 20-month-old boy presenting with fever, hepatosplenomegaly, pancytopenia and coagulopathy. An initial diagnosis of kala-azar was rejected because of absence of biological inflammatory syndrome and absence of leishmania in bone marrow. Specific serology for visceral leishmaniasis become positive after a week and he was treated with N-methylglucantime for 21 days.
