A 42-year-old man from the USA with acquired immune deficiency syndrome was evaluated for a pruritic eruption on his face, arms and trunk There were numerous dome-shaped, flesh-coloured papules ranging from 0.5 to 3 mm in size, with central excoriations. The lesions were most heavily distributed in areas of high follicular density. Blood cultures grew Cryptococcus neoformans. Although antifungal therapy was started, the patient died 3 days after admission.
