A combined detection and drug susceptibility testing of mycobacterium tuberculosis in smear positive sputum samples using biphasic vial radiometric method

A combined detection and drug susceptibility testing of tubercle bacilli in 24 smear positive and one smear negative sputum samples was compared using radiometric biphasic vial system and the conventional microbiological method. The biphasic vial system, had solid Lowenstein-Jensen medium as the nutrient support, while 14C-U-acetate served as a radiolabel substrate and liquid scintillation counter was the detecting device for 14CO2 produced. The mean detection time for the sputum samples was 10.5 and 21.7 days by radiometric and visual method respectively. The primary growth on the surface of the radiometric vials was subjected to the drug susceptibility testing when the vials showed 10,000 cpm. The reportable drug susceptibility results were available within 4-7 days by the radiometric method and 21-28 days by the visual method. An overall agreement of 95.4 per cent was observed between the two methods. The sensitivity and specificity for isoniazid were 1.0 and 0.98; rifampicin 1.0 and 0.96; streptomycin 0.95 and 0.88; and ethambutol 0.94 and 0.91 respectively. The total time for combined detection and drug susceptibility testing of the 25 sputum samples by radiometry ranged between 12-34 days with a mean of 20.5 days. To obtain a comparable result the visual method required a mean detection period of 21.7 days with additional 21 to 28 days for drug susceptibility testing.
