Anterior cervical interbody fusion for the syndrome of acute central cervical spinal cord injury clinical and experimental studies

In autopsy specimens from 2 patients with the syndrome of acute central cervical spinal cord injury, the pathological lesion was localized mainly in the gray matter of the cervical spinal cord with only slight changes in the white matter. The marked compression of the cord was confirmed macroscopically and roentgenologically by the posterior bulging of the intervertebral disc in 1 patient and by the ossified posterior longitudinal ligament in another one. An experimental study was made using 12 adult mongrel dogs and the mechanical compression of the spinal cord was studied. Experimental compression to the cervical spinal cord was carried out using small pieces of laminalia and similar changes noted in the cord at autopsy were obtained in 6 out of 12 dogs. Direct compression of the spinal cord at the time of injury may contribute to production of the syndrome. Seven patients with symptoms similar to those seen at autopsy were treated by means of anterior decompression and spinal fusion. A satisfactory recovery was obtained in all patients, thus demonstrating the effectiveness of the surgical approach.
