Clinical applications of endoscopic neodymium yag laser contact irradiation of gastric tumors

Contact irradiation of Nd-YAG laser was applied to 3 cases of IIa type of early gastric carcinomas and 6 cases of borderline lesions of the stomach with a new type of endoprobe (Table 1). One of three IIa lesions declined operation and two were heistated by the reason of severe complication. For the contact irradiation, the endoprobe, made of ceramics, was used (Figure 1). In one of 6 cases of borderline lesions irradiated, residual tumor cells were observed after the first irradiation, and also a case of IIa lesion, cancer cells were found in biopsy materials at 2 months after Nd-YAG laser irradiation (Figure 3). To these two cases the second irradiation was tired. However, tumor cells were eradicated in the others. Up to 12 month after the irradiation, endoscopic examinations have been repeatedly performed with aids of biopsy study, and no tumor cells were found in the period. From these results, the contact irradiation of Nd-YAG laser is likely to be effective for treatment of borderline lesions and/or early cancers of the stomach. But clinical applications of this treatment to early gastric carcinomas should be limited to the cases with no indication of surgical operation.
