Combined treatment of laryngeal cancer with pre operative radio therapy under hyperbaric oxygenation conditions

A comparative study was made of the efficacy of radiotherapy of laryngeal cancer. Stages II-III using mean dose fractionation (3.3 Gy [Gray] 3 times a wk up to the summary dose of 33 Gy) in the air and under hyperbaric oxygenation (HBO) conditions. Thirty patients were entered into the main group (irradiation under HBO conditions) and 34 into the control group. In both groups preference was given to resection of the larynx: after irradiation in the air it was performed on 26 out of 34 patients, under HBO conditions on 25 out of 30 patients. Preoperative irradiation is well tolerated by patients with laryngeal cancer; it does not complicate subsequent surgical intervention or produce a negative influence on the healing of postoperative wounds. Irradiation under HBO conditions as compared to irradiation in the air has some advantages: a growing degree of tumor damage by irradiation with the weakening of local radiation reactions and later on the improvement of immediate therapeutic results. While planning resection of the larynx, particularly in patients with Stage III, preference should be given to irradiation under HBO conditions.
