Effect of hematocrit and viscosity on coronary circulation and myo cardial oxygen utilization

Hematocrit (Hct) elevation provokes myocardial ischemia in various clinical conditions. To clarify the effect of Hct and blood viscosity on coronary hemodynamics and O2 utilization, isovolemic exchanges of blood with plasma or packed red cell were performed in 10 open-chest anesthetized dogs. Hct was varied in the range of 20-60%. Coronary blood flow and cardiac output changed inversely with Hct. Coronary vascular resistance and blood viscosity elevated in logarithmic relation to Hct. Coronary vascular resistance and blood viscosity revealed linear relationship; the coronary vascular hindrance (resistance/viscosity) remained constant for the Hct range of 20-60%. The arterial O2 content increased linearly with the elevation of hematocrit and the myocardial O2 extraction rate was unchanged. The systemic O2 transport rate increased with Hct. The left ventricular O2 transport rate varied little, although the left ventricular O2 transport at medium Hct was slightly higher than at low or high Hct. The myocardial O2 consumption remained constant at each Hct, and the efficiency was high at low Hct in the face of the elevated cardiac output. The increase in Hct elevated the blood viscosity and played a significant role in determining the coronary vascular resistance; this could induce a disturbance in coronary microcirculation especially in pathological conditions.
