Effect of rehabilitation treatment in sanatorium on selected hemodynamic parameters in patients after acute myocardial infarction

In 12 men after acute myocardial infarction the heart rate, arterial blood pressure, pulmonary artery wedge pressure (measured with a Swan-Ganz catheter), minute heart volume (by thermodilution) and oxygen saturation of capillary and mixed venous blood were determined. These determinations were done twice, before and after completion of a 28-day sanatorium rehabilitation treatment in Naleczow - spameasuring these values at rest and in the last fourth minute of isometric effort (handgrip), which included compression exerted on a dynamometer with a strength equal to 30% of the maximal strength, and in both studies, before and after rehabilitation treatment the same workload was used. In the light of the obtained values of these haemodynamic paramaters the cardiac index, the ejection volume index, the total peripheral resistance and left ventricular work were calculated from the difference in the oxygen saturation of arterialized capillary blood and mixed venous blood the arteriovenous blood oxygen saturation difference was determined. It was found that in patients after acute myocarial infarction after rehabilitation treatment in sanatorium the resting systolic arterial pressure decreased, and the remaining haemodynamic parameters were not significantly different before and after the rehabilitation. On the other hand, during isometric effort the test done after rehabilitation showed that in relation to the values before the rehabilitation the systolic and diastolic arterial pressure was decreased, the heart rate was reduced, the pulmonary wedge pressure was lower, the total peripheral vascular resistance was decreased, left ventricular work was lower and the value of the ejection volume index was higher. These changes show a favourable effect of physical rehabilitation treatment on the peripheral circulatory system and on the function of the left ventricle in patients after acute myocardial infarction.
