Endoscopic cytology of the upper part of the digestive tract

Cytological examination forms since 1979 a regular part of endoscopy of the upper part of the digestive tract. The authors evaluate 917 cytological examination of brush and 422 cytological samples prepared by print of bioptical specimens bioptic specimens. The group is formed by 277 patients with tumours of the stomach or oesophagus and 1062 with a benign disease, mostly a gastric ulcer (502 subjects). In brushs the sensitivity of cytology was 62.8% in gastric tumours, oesophageal tumours only 57.1%. The specificity of the cytology is 98.3%. Inclusion of cytology into the routine endoscopic programme increases the detection rate of cancer by 6.7%. 15.7% of the brushs could not be evaluated from the cytological aspect. The introduction of print cytology reduced the number of cytological findings which could not be evaluated; when the two methods are combined only 7.8 cytological findings cannot be evaluated. At the same time also the sensitivity of the two cytological methods increased in patients with gastric cancer to 81.2%. By cytological and histological examination a gastric or oesophageal tumour was correctly diagnosed in 94.5% of the patients. Endoscopic cytology is recommended in addition to aimed biopsy as an important supplementary method for the endoscopic diagnosis of diseases of the stomach and eosophagus.
