Endoscopic ethanol injection for the control of upper gastrointestinal bleeding

The results of endoscopic injection of absolute ethanol for the control of upper gastrointestinal bleeding are presented. In a retrospective study of 20 patients with active bleeding the rate of initial hemostasis was 65.2%, rebleeding 26.6%, mortality from bleeding 25%, and overall mortality 45%. Success rates varied with the type of bleeding lesions, high in chronic gastric ulcers or oozing lesions and low in acute stress ulcers associated with serious medical problems or spurting bleeders. Although experience to date has established the usefulness of endoscopic bipolar electrocoagulation in massive bleeding, local injection of absolute ethanol is a simple and easily applicable technique that has been shown to be convincingly effective in selected patients.
