We report a case of endoscopic resection of early gastric cancer, type IIc. A 79 year-old male, who had been suffered from diabetes mellitus and Parkinson's disease for several years, was admitted to our hospital because of the aggravation of Parkinson's disease. Gastrofiberoscopy on admission showed a small depressed lesion with hemorrhage at greater curvature of the gastric antrum. The biopsied specimen showed tubular adenocarcinoma. There was no operative indication because of the complication such as diabetic nephropathy and Parkinson's disease, and his advanced age. So endoscopic resection was carried out. The method is as follows; 3 ml of saline is locally injected in the submucosa at the oral side of the cancer. Artificially, the depressed lesion becomes protruded one. Using 2-channeled fibroscope (Olympus GIF-2T), the lesion can be pinched up by grasping forceps, and resected with diathermic snare as same as in polypectomy. The resected specimen shows early gastric cancer, which is localized only in the gastric mucosa, with the size of 7 mm. The lesion could be completely resected. There are a few reports of endoscopic resection of depressed type early gastric cancer. Some literal considerations about the indication of this therapy are done.
