Endoscopic urethrotomy for sclerotic stenosis

The possibility of clinical application of internal endoscopic urethrotomy for the treatment of sclerotic urethral disorders is discussed. Treatment results obtained in 51 patients with strictures of the urethra are presented. The mean age of the patients was 61 years. Patients with iatrogenic strictures in the membranous-prostatic part of the urethra made up the most numerous group. The patients were operated on, using a clinically-verified procedure, strict adherence to the technique of the operation and postoperative examination, and appropriate postoperative management being major prerequisites of success. The best effect was obtained in cases of primary strictures where prolonged bougienage was avoided. The treatment was less successful in cases of recurrent strictures and those with many years' history of bougienage. The total recurrence-free operation rate was 84%. Two case reports are presented as examples. In both cases, internal endoscopic retrograde urethrotomy was performed. No recurrences were recorded over 3 years of follow-up. Internal endoscopic urethrotomy is offered in addition to open surgical methods as a means of successful treatment for all short-length urethral strictures.
