In the later stages of the National Malaria Eradication Program [NMEP] occurrence of a positive case in maintenance and consolidation phase areas was a major epidemiological event. It was necessary to investigate in detail such a positive case and to classify it according to its origin. The main purpose was to know with certainty whether local transmission has been resumed or not, and to institute measures to liquidate such a focus of transmission to maintain the malaria free status of the community in maintenance and consolidation phase areas. Under the Modified Plan of Operation of the NMEP, the immediate goal is not eradication of malaria. The occurrence of an individual positive case has lost the importance as an epidemiological event. Situations arise where in areas < 2 API cases/annum or even in areas 2 API above, individual foci of intense transmission are set up recording the increased number of positive cases especially Plasmodium falciparum. It becomes essential to know the genesis and source of such a focus and to plan immediate remedial measures. The present communication highlights these aspects and describes the process of epidemiological investigations under such situations.
