Duties of a pediatrician include early diagnosis of allergic diseases (bronchial asthma, respiratory allergoses and allergic dermatoses) and directing patients with such diseases to a specialized hospital. About 3% of children in Leningrad, USSR suffered with these diseases (2% with allergic diseases of the respiratory tract and over 1% with allergic dermatoses). About 20% of them required prolonged hospital observation. Children in their 1st 3 yr of life required monthly examinations mainly for food sensitization accompanied by allergic dermatosis. Hospital duties included detection of allergenic products in the diet, the elaboration of a nonspecific desensitizing therapy and recommendations for a special diet regimen and keeping living quarters of such children free of dust and other measures. The primary prophylaxis of allergic diseases was conducted by examination of healthy children with a high risk of allergosis formation. Risk factors were mainly the presence of allergic diseases in relatives and high antigen loading in the late antenatal and early postnatal period stimulating the synthesis of IgE. Frequency of allergic diseases in high risk children was 10 times higher than in those with favorable inheritance. Therapy for these children included the exclusion of obligate food allergens from the mother during the 2nd half of pregnancy and during the period of lactation and recommendations of maximum natural feeding of the child (up to 1 yr) with exclusion of juices and additives with high sensitizing activity. For treatment of intercurrent diseases antibacterial preparations were administered sparingly.
