Intrathecal tetanus hyperimmune human gamma globulin in the treatment of tetanus

Four hundred and twenty eight hospitalized patients of tetanus, were studied from January, 1980 to April, 1985. The severity of the disease was graded into mild, moderate and severe tetanus after clinical evaluation. All patients received antibiotics, sedatives like diazepam and intrathecal human tetanus immune gammaglobulin (TIGH) besides good nursing care. The patients were divided into two groups. Group A received 500 IU, 750 IU and 1000 IU of TIGH and Group B 750 IU, 1500 IU and 3000 IU of TIGH in the age groups, 1-3, 4-7 and 7-12 years, respectively. The survival in Group A was 86% and in Group B 93%, a difference which was statistically significant (p < 0.05). It is concluded that TIGH is a useful adjunct to standard tetanus therapy and a dosage regimen of Group B should be employed.
