Labetalol in patients with hypertension and varying degrees of renal impairment

Hypertensitive patients (7 male, 6 female) with a mean age of 52 .+-. 12 yr were treated with Labetalol for a mean period of 10.5 mo. Five patients had essential hypertension and 8 had renal disease. Four patients had severely impaired renal function (GFR [glomerular filtration rate] < 10 ml/min), 4 had moderate impairment (GFR > 60 ml/min) and 5 had normal function (GFR > 100 ml/min). Blood pressure, GFR, ERPF [renal plasmal flow], urea and electrolytes were assessed before commencing Labetalol and at 2 wk intervals thereafter. Labetalol as monotherapy controlled blood pressure in 12 patients and in combination with Natrilix in the remaining patient. There was no significant difference, at any stage during the study, between GFR and ERPF estimations taken before commencing Labetalol and those taken subsequently while on Labetalol. Pre-trial GFR and ERPF were 76.4 .+-. 49 ml/min and 391 .+-. 226 ml/min, respectively, and end of trial values were 76.8 .+-. 48 ml/min and 444 .+-. 261 ml/min, Labetalol is a safe effective antihypertensive agent in patients with hypertension-associated renal impairment.
