Labor in older pregnant women

Ginekologia Polska 50(4): 313-316

High perinatal mortality rate of newborns in primiparous women older than 31 yr urged a change of opinion concerning the conduction of labor in such cases. To compare attitudes and to choose the better, 2 periods were compared. During 1972-1974 more conservative (awaiting) procedures were used. In this period in 73 delivering primiparas (older than 31 yr) the percent of cesarean sections was 10.9%, forceps 31.5% and breech delivery 6.9%. During 1975-1977, when treatment was more active in 202 delivering primiparas, cesarean section was used in 32.3%, while forceps delivery was only 8.9% and breech delivery, 0.5%. The mortality rate decreased from 5.4% in the 1st period to 0.5% in the 2nd; the number of perinatal injuries of the newborns was also markedly decreased.
