Long term bronchial arterial infusion chemo therapy

In an exploratory thoracotomy on locally advanced unresectable [human] lung cancer, the technique of continuous intubation of a catheter into the bronchial artery supplying the tumor was designed. In 5 cases, infusion of anticancer agents was performed through the catheter intermittently or continuously for a period of 30-71 days. The local anticancer effect was very remarkable compared with that of i.v. injection of the drugs. However, general side effects were significantly less. In 2 autopsied cases who died of distant metastasis, almost all of the cancer cells in the primary lesion were histologically degenerated and necrotized. The level of adriamycin was measured in circulating blood after the injection from the catheter indwelled in the bronchial artery. The maximum level of adriamycin after injection from the catheter was several times as low as after i.v. injection. Adriamycin is probably absorbed locally; generally, the degree of side effects is lower. To observe the effective distribution of the drugs, 99Tc-MAA [macroaggregated albumin] was introduced through the catheter. During the early period of therapy, the isotope distributed widely along the bronchial trees, but on progress of the therapy it localized gradually in a limited area, probably because of A-V [arteriovenous] shunt formation due to vascular damage by the drugs. The proper dose of the drugs and the duration of the therapy are still under investigation. This technique seems very useful in the local therapy of lung cancer.
