Operative treatment of primary lung cancer

A total of 2160 patients with primary lung cancer were treated over the past 26 yr. The operability rate was 52.3%, the resectability rate was 42.7%. Pneumonectomies were performed in 308 patients, extended pneumonoectomies in 260, partial resections in 349 and thoracotomies in 206. The postoperative case fatality rate over those years was 9.3%. Some 30% of the patients survived for more than 5 yr. The best results were obtained in cases of squamous cell carcinoma (33.6% of the patients 5 yr or more). The 5 yr survival rate was 62.7% in stage I cancer, 20.8% in stage II and 19.4% in stage III. The main treatment method in primary lung cancer remains early operation. To be able to treat primary lung cancer more successfully, methods of early diagnosis and better methods of combined treatment need to be developed.
