Post operative acute renal failure of gastro intestinal tracts early detection and management of acute renal failure with special reference to multiple organ failure

There were 10 cases of postoperative acute renal failure of 429 cases with abdominal surgery from Dec. 1977-Aug. 1981. Most of these cases were due to peritonitis, including anastomotic leakage. The survival rate of acute renal failure was 50%. All the cases with acute renal failure developed multiple organ failures. Prognosis became poor with the increase in the number of organ failures. Free-water clearance was a useful marker for the early detection of acute renal failure. Hyperosmolar nonketotic diabetic coma was a dangerous complication when using i.v. hyperalimentation. Acute renal failure apparently was the sign of multiple organ failure, since all the cases with renal failure developed multiple organ failures. Surgical drainage and prophylactic therapy for circulatory disturbance were the main types of therapy for multiple organ failures due to infection.
