Post transfusion thrombocytopenia connected with alloimmunization with the zw ap a 1 platelet antigen
Clinical, haematological and serological data are reported of the first Polish case of post-transfusion and thrombocytopenia developed on the 6th day after blood transfusion for strumectomy in a 39-year-old Zwa(PlA1)-negative women sensitized by previous pregnancies and blood transfusions. Alloantibodies against platelets of anti-Zwa(PlA1) specificity belonging to IgG class were demonstrated and identified by means of the antiglobulin fluorescence test. Despite serious haemorrhagic complications complete spontaneous recovery occurred after about 30 days from the onset of the disease. Normal platelet count was observed during over one-year follow-up after recovery. At the same time, a secondary immunological response was found to be c antigen from the Rh system of the erythrocytes transfused during the operation. There was mild destruction process of these erythrocytes. Antibodies to Zwa(PlA1) similarly as anti-c anti-bodies were repeatedly demonstrated during this follow-up. The authors discuss the hypotheses explaining the pathogenesis of this unusual and interesting immunological complication of blood transfusion and draw several practical conclusions.
