Post traumatic psychoses a retrospective study of 18 cases

The authors found 3.4% of post-traumatic psychoses in 530 cases of brain injury, followed up for one to ten years, and made a critical review of the literature concerning this unusual post-traumatic personality disturbance. Most psychotic patients appeared to be young males. Paranoid and schizophreniform disorders were the most frequent type of psychotic decompensation. Paranoid disorder predominated in chronic psychoses and schizophrenic disorders in acute delusions. Delusion of persecution has been the most frequently observed symptom. The other main symptoms were deep disturbances of the mood, especially of manic type, and hetero- and auto-aggression. The type and the severity of the injury appear irrevelant factors, the temporal site of the lesion as a slightly more important factor and the presence of previous psychological disturbances as a very one being found in 83% of the cases. Psychosis disappeared in half the caes described as acute delusion. On the other hand, the prognosis of chronic psychosis appeared severe. (46 ref).
