Postinfarction aneurysms of the heart 10 years of our own clinical experience

Forty three patients with postinfarction aneurysms of the heart aged between 36 and 76 years were analysed. An analysis included: history, X-ray examination, ECG, and coronography and ventriculography in 15 patients (group A). The group A was compared with the group of patients with myocardial infarction without aneurysm (group B). Ventriculography of the patients of group A has shown akinetic areas in the apex area in 7 cases, anterior wall in 1 case, and the whole left ventricule in 4 patients as well as dyskinetic areas on the antero-lateral surface in 3 patients. Complete or critical stenosis of the intraventricular anterior artery were seen in 10 patients while stenosis in the main trunk of the left coronary artery by 50% in 2 patients. Critical lesions to more than one coronary artery were seen in 7 patients. Collateral circulation developed in 5 patients of the group A: P-type in 2, M-type in 1, F-type in 1, B-type in 1 patient. In the group B the following types of the collateral circulation were found: Q-type in 6, P-type in 3, O-type in 1, M-type in 2, N-type in 2, and B-type in 1 patient. Coronography performed in the patients with aneurysms has shown increased sclerotic process in the coronary arteries and more frequently the absence of the collateral circulation than in the control group.
