Preliminary observation of histologic, histochemical and electron microscopic examinations of endometrium after implantation of a contraceptive prosthesis

This paper investigated the condition of the endometrium in 14 women wearing IUDs. These women ranged in age from 34-53. Endometrial biopsies were done within 1 week of the menstrual flow in these women who had worn IUDs for between 7-16 years (average period was 10 years). For observation, the authors used an ordinary optical microscope and the histochemical method, namely PAS stain and alkaline phosphatase stain. The results were compared with those obtained from normal women and women with irregular menstrual cycles, all of who did not wear an IUD. It has been observed that, due to IUD placement, the secretory function of the endometrium has been reduced and the activity of the alkaline phosphatase has been weakened. For further study, electronmicroscopy was also used. It was found that IUD insertion resulted in a decrease in the secretory function of the glandular epithelium of the endometrium. There were other unfavorable effects. No other significant reactions were observed. Based on the abovementioned observations, the authors conclude that longterm insertion of the IUD has no appreciable ill effects on the user. However, readers should bear in mind that the number of cases investigated is still small. Finally, in connection with the contraceptive mechanism of action of the IUD, it may be that the decrease in the secretory function of the endometrial glands due to mechanical irritation of the IUD strongly influences the implantation of the fertilized ovum. Thus, contraceptive benefit is the result. (author's modified)
