Preservation of mueller's muscle in the levator shortening procedure

In congenital blepharoptosis, the levator palpebrae muscle is largely damaged, but Mueller's muscle, which consists of smooth muscle and receives sympathetic nerve supply, has an intact function. In our procedure of levator shortening, insertion of thin plastic film between the palpebral conjunctiva and Mueller's muscle facilitates the identification and dissection of the levator aponeurosis, Mueller's muscle and conjunctiva. Mustarde's split level lid resection with levator tacking and preservation of Mueller's muscle can be easily accomplished by this simple maneuver.
