Principles of surgical treatment of chronic pancreatitis

Indications for the surgical treatment of chronic pancreatitis were discussed. An outline for the classification of pancreatitis was presented. During 15 yr of clinical experience, 400 patients with chronic pancreatitis were observed and 232 operations were performed. According to the etiology, pathogenesis and the use of several diagnostic methods (fibroscopic transpapillary contrasting of the pancreatic ducts, selective vasography of the pancreas and laparoscopic cholecystocholangiography), 2 variants of the disease were distinguished: primary pancreatitis (with degeneration of the pancreatic parenchyma) and secondary pancreatitis (accompanied by hypertension of pancreatic ducts and often resulting from cholelithiasis and choleystitis). Surgical treatment of primary pancreatitis was rarely indicated while surgical treatment of secondary pancreatitis was indicated in almost every case. Surgical treatment of secondary pancreatitis not only included cholecystectomy and unblocking of the ducts, but also the creation of a free outflow of bile and pancreatic juice into the duodenum by papillocholedochoplasty or a pancreatodigestive shunt.
