During electrophysiological examinations in 5 of 47 patients (11 per cent) with WPW syndrome and coexisting tachycardia, direct conversion of re-entry tachycardia into atrial fibrillation and reciprocally was observed. Re-entry tachycardia spontaneously converted into atrial fibrillation in 2 cases and in another 3 was provoked by fast atrial stimulation used for discontinuation of tachycardia circuit. In this group of patients atrial extrasystole occurring in the course of re-entry tachycardia initiated paroxysmal atrial fibrillation. On the contrary, conversion of atrial fibrillation into re-entry tachycardia was recorded in 3 cases. The presence of Kent's bundle anterograde block was the coexisting factor for conversion of one tachycardia into the other. Studies indicate that the conversion of one tachycardia into the other in a course of WPW syndrome is not a rare phenomenon. Recordings of atrial electrocardiograms can be very useful in some cases, in diagnosis of described phenomenon, difficult to diagnose based on normal ecg recordings.
