Role of intra myometrial prostaglandin e 2 in management of 3rd stage of labor

A total of 100 women who delivered normally were divided into 2 groups (A and B), group A being given prostaglandin E2 (PGE2) 0.5 mg intramyometrially at the fundus of the uterus at the time of crowning of head, and group B being given methergin 0.25 mg, i.v. A preliminary clinical study was done to compare the effect of intramyometrial PGE2 0.5 mg and methergin 0.25 mg in the 3rd stage of labor, to minimize the blood loss and to reduce the time interval of placental separation and expulsion. In group A the placenta was expelled within 3 min in 92% of cases while in group B the expulsion occurred only in 60% of cases (P < 0.001). The total blood loss in the 3rd stage was less in group A, the average blood loss (100 ml) being reported in 88% while in methergin group (B) it was 52% (P < 0.005). In the methergin group, 1 woman had an hourglass contraction of the uterus requiring manual removal of the placenta. She also had post partum hemorrhage in the PGE2 group (680 ml blood loss); no serious side effect or complication was observed. PGE2 may be used effectively and safely intramyometrially, as a prophylactic measure, in the management of the 3rd stage of labor.
