Solitary ulcer of the rectum report of a case and a review of the japanese cases

A solitary ulcer of the rectum of a 70-year-old male with complaint of profuse anal bleeding was presented. He had not easily controllable diabetes mellitus and hypertension. Barium enema and proctosigmoidoscopic examinations revealed relatively a large ulcer in the anterior wall of the lower rectum without any abnormal lesion in other portion of the colon. Histologic examination showed a mucosal ulceration without granulomatous or other specific pathological features. The ulcer was resistant to various kinds of conservative treatment. Three months after the onset of the disease he suddenly died of cerebrovascular accident. We speculated that the cause of the ulcer in this case was based on the regional ischemia due to severe diabetic vascular changes associated with hypertension of an elderly patient.
