During the last 30 years the number of patients with pulmonary carcinoma admitted to Rijeka University Hospital - the Chest Department at Icici has been continuously increasing. From 1953 to 1974, 21.601 patients were hospitalised: with carcinoma 1.724(8%); from 1974, to the end of 1984, 14,631 patients: with carcinoma 1.668 (11.4%) 22.112 bronchologic examinations were performed in the same period of 30-years and from 1954 to 1974 carcinoma was found in 15.7% and from 1974 to 1984 in 21.6% of hospitalised and out-patients bronchoscopied patients. The first thoracotomy in our region was performed in 1953, and later more than 2,000 thoracotomies were performed for various pulmonary diseaes. Bronchoscopy combined with citology of broshings, aspirated secretion and bronchoalveolar lavage may yield a prompt diagnosis. In the case of peripheral or localized disease resection of the entire involved area is the best treatment. In inoperable cases (to this day more than 70%) reasonable palliation could be obtained with radiation and chemotherapy. Finally, the role of diagnostic procedures FOB, X-ray studies, citology and multidisciplinary approach is discussed on the basis of 3-years experience.
