Incipient gastric cancer is an evolutive phase of gastric cancer. It is differentiated from advanced gastric cancer on the basis of precise morphologic criteria that do not have in view the size or affection of the lymph nodes. A diagnosis of incipient cancer can only be done following examination of the resection specimen. The diagnosis is of importance as it is the only means of improving radically the prognosis of gastric cancer. The existence of elements such as stable macroscopic morphologic types, prolonged evolution, clinical symptomatology, precancer affections and immunologic markers should facilitate the diagnosis of incipient cancer, and endoscopic examinations increase its rate of detection. Critical analysis of these factors is of relative practical importance when confronted with the clinical symptomatology and a precancer state. Outpatient follow-up of patients with gastric ulcers, with compulsory biopsy, irrespective of the endoscopic aspect, increasing endoscopic examinations of symptomatic patients and the hope of immunologic determinations in screening procedures might improve the early detection of incipient gastric cancer.
