The history of the emiro de lima's approach to the ethmoid sphenoid and frontal sinuses

Described for the first time in 1936, the transmaxillary approach (or transantral) to the ethmoid, sphenoid and frontal sinuses proposed by Ermiro de Lima presents, as its main characteristic, a meticulous systematization which makes this surgery a disciplinated, adequate and safe procedure. Despite the fact that many authors have used the transantral approach before, this access proposed by Ermiro de Lima (1936, 1943) is original for: (a) Its systematization with clear and safe anatomical landmarks for emptying and drainage of the sinuses operated on, (b) Its approach to the frontal sinus through an exclusively transantral access, (c) Its preservation of the nasal turbinates and the structures of the middle meatus. The paper is divided in three parts: (I) On the origin and characteristics of the technic itself, (II) On the Ermiro de Lima's original contribution, (III) On the spreading of technic and its present status.
