In order to study the epidemiology of diabetes in 40-69 year old people in Taipei City, 3,100 persons among 564,965 middle-aged and aged persons were selected from 16 districts of Taipei City from December 1984 to March 1985. They were sampled randomly. A total of 2206 persons were examined (71.2%) and 133 cases of diabetes mellitus were diagnosed according to the criteria of WHO (1980) from the persons with high fasting capillary blood glucose (> 105 mg/dl) and/or with a personal history of diabetes and of persons from diabetic families. The results were: 1. The prevalence rate of diabetes among 40-69 years old person in Taipei City was 6.0%. The prevalence rates among persons of 40-49 years old (25 cases), among 50-59 years old (56 cases) and 60-69 years old (54 cases) were 3.0%, 6.5% and 9.3% respectively. It was increased with aging. Among them, 58 cases were known diabetics and 75 were newly diagnosed. There were 30 cases with a family history of diabetes (22.6%). Only 35 persons or 26.3% had regular visits to the clinics. The prevalence rate in males and females were 5.5% and 6.4% respectively. It is lower in males than in females. 2. The body weight of the diabetic patients is greater than that of normal persons and those with impaired GTT. The incidence of obesity in diabetic patients was 37.5%. The incidence is higher than that of normal people. (22.2%). It is more common in females than in males and it is one of the prevalent factors. 3. Diabetes is complicated with hypertension in 27.1%, higher than that of the normal person. (12.5%, P < 0.05) 4. Among all the cases with diabetic ophthalmological complications, there were 40 cases (30.1%) with cataracts, and 57 cases (42.9%) with diabetic retinopathy. In the 57 cases, there were 40 cases (30.1%) with background diabetic retinopathy, 8 cases (6.0%) with preproliferative diabetic retinopathy, 9 cases (6.8%) with proliferative diabetic retinopathy. 5. It is much easier to control diabetes in patients who are knowledgeable about diabetes and the complications of hypertension, obesity and retinopathy were less in such knowledge-able diabetics and those who took regular check upsat the clinics. It is useful to have a routine check of blood sugar to make early diagnosis and prescribe treatment. Diabetes education is important in prevention of the complication of diabetes.
