Triplet births. 24 cases collected over 10 years

From Jan. 1, 1969 to Dec. 31, 1978 67 children, the issue of 24 triplet pregnancies were studied. Five children died before admission. The mean age of the mothers was 30.2 yr and 15 of the 24 mothers were multiparous. There was a family history of multiple pregnancies on the maternal side in 16 of the 24 mothers and 9 had ovulation induced by treatment with gonadotropins and/or with Clomiphene citrate. Sixty babies were born through the vagina but 17 of these only after obstetrical maneuvers necessitated by abnormal presentation. Twelve children were born by Cesarean (2 for acute fetal distress; 1 for toxemia of pregnancy and 1 case for cervical dystocia). Most [87.5%] were born prematurely (the mean gestational age being 36.2 wk). The mean birth weight was 1800 g (the range was from 960-2700 g). Infants (20, 40.2%) had intra-uterine growth retardation. The sex ratio was equal, 36 boys and 36 girls. Nineteen infants had signs of respiratory troubles: 5 cases of hyaline membrane disease; 14 cases of fetal asphyxia syndrome which were possibly brought about by too long an interval between the deliveries of the infants. Infants (7, 9.7%) died. There were 3 cases of in utero death because of acute fetal distress and 2 deaths during treatment. One case died of sepsis and 1 case died of hyaline membrane disease complicated by ventricular hemorrhage. Two of the children from 1-10 yr old showed psychomotor retardation with development quotients of < 80 and 3 had speech difficulties. In children under the age of 1 yr, the prognosis seemed favorable but they were not observed for sufficiently long. Multiple pregnancies are high-risk pregnancies. The diagnosis should be made early to avoid prematurity and high quality care must be available in labor and in the neonatal period in order to improve the neonatal prognosis.
