Variability of isoniazid pharmacokinetics

The pharmacokinetics of isoniazid was examined in 236 new cases of destructive pulmonary tuberculosis. Upon rapid i.v. injection of isoniazid in a dose of 15 mg/kg, the differences in the groups of rapid and slow acetylators remained unchanged. Comparative analysis of the pharmacokinetics of isoniazid and the time course of acetyl isoniazid excretion in the group of rapid acetylators allows one to distinguish the patients who are homozygotic as regards the gene that controls the acetylation. The time course of the decrease of isoniazid blood content and the time course of excretion of the unchanged drug and acetyl isoniazid with urine may serve as criteria of disturbing the patients into 3 pharmacokinetic types: rapid and slow acetylators, and slow acetylators with rapid excretion of active isoniazid.
