A case of choroidal malignant melanoma

Folia Ophthalmologica Japonica 40(7): 1622-1627

We present the case of a 77-year-old man with choroidal malignant melanoma containing lipid droplets with melanosomes in situ in the tumor cells. The moderately vascularized tumor, mushroom-shaped and blackish in color, was mainly composed of epithelioid cells. Oil red O staining for lipid was positive. Mitosis was scarcely observed. Most of the larger lipid droplets were membrane-bound; the others had no limiting membrane, with melanosomes in situ. These findings suggest that lipid-containing cells, some of which are balloon cells, are not macrophages but tumor cells. Fatty change in tumor cells might be a kind of harbinger of cell death, although no necrosis was observed in this tumor. The smaller melanosomes were usually ellipsoidal, with internal lamellar structure, while the larger ones were more rounded with granular internal structure containing spotty and granular melanin deposits. The former were morphologically consistent with eumelanosomes and the latter with pheomelanosomes, though they were in the tumor cells.
