A case of fibroma of ovary showing elevated serum ca 125 values

CA-125 is a specific tumor marker for epithelial cancer. Recently, CA-125 has been reported to be positive in some cases of benign diseases. We report here a case of fibroma of ovary showing elevated serum CA-125 value which as suspected to have ovarian cancer before operation. Our patient, a 66-year-old housewife, received medical examination at our Department of Obstetrics and Gynecology with main complaint of weight loss and feeling of hypogastric fullness. Our examinations revealed abundant hydrothorax and ascites; the uterus was of normal size but with palpable solid tumor of child's head size in the right adnexal region. Moreover, the serum CA-125 was as high as 4,000 U/ml, and image diagnoses were made by CT and ECHO. From the abovementioned findings and laboratory data, laparotomy was done under the suspicion of ovarian cancer. The specimens obtained by operation were pathologically diagnosed as ovarian fibroma. The clinical course, serum CA-125 value, and immunohistochemical localization of CA-125 in this case are reported. The serum CA-125 was as high as 4,000 U/ml before operation but fell below the standard value at about 4 weeks after operation. Immunohistochemical findings of the surgical specimens revealed no tumor tissues with localized CA-125. The elevated serum CA-125 was presumed to be due to peritoneal reactions to the ascites rather than to its production in the tumor itself.
