A case of intraocular pressure elevation caused by mydriasis after laser iridotomy

The patient was a 74-year-old male who underwent argon laser iridotomy in both eyes for chronic angleclosure glaucoma, diagnosed at initial examination. The intraocular pressure (IOP) in both eyes rose to a high level of 40 mmHg due to post-operative dilatation of the pupils, but the iris holes remained open and angle closure due to mydiasis was not observed at that time. As to the mechanism of IOP elevation due to mydriasis in this patient, it is suspected that, because of the underlying condition of severe trabecular dysfunction, copious amounts of iris pigment or plasma protein, discharged into the anterior chamber due to dilatation of the pupil, aggravated the impaired outflow of aqueous humor in the trabecular meshwork, causing the IOP to rise markedly.
