A case of left tension pneumothorax associated with eosinophilia in the massive pleural fluid in the lung tissues and peripheral blood

A 72-year-old male with left tension pneumothorax associated with eosinophilia in the massive pleural fluid, in the lung tissues and in the peripheral blood is reported. A pleural biopsy revealed eosinophilic infiltrates and Charcot-Leyden crystals in the pleura. The trans-bronchial biopsy showed eosinophilic infiltrates in the alveolar septa. There was also noted eosinophilia in the bronchoalveolar lavage fluid. The association of pleural eosinophilia, pulmonary eosinophilia, peripheral blood eosinophilia and tension pneumothorax was considered extremely rare. Eosinophilic granuloma was excluded. The etiology of these manifestation remains unknown.
