Corticosteroid reaction of the adrenal glands in patients with sepsis before and after hemoperfusion

The studies were performed on 23 patients with sepsis who were subjected to sorption detoxication. Glucocorticoid blood spectrum was studied prior to and following hemosorption. The results obtained made it possible to conclude that hemosorption has a stimulating effect on the adrenal function. The stimulating effect is, possibly, connected with corticosteroids elimination from the blood. However, in seriously-ill patients, in whom the initial corticosteroid level is low and the functional capacities of the adrenal glands are exhausted the development of severe complications is possible. Therefore, corticosteroid substitutioanl therapy seems expedient in seriously-ill patients with unstable hemodynamics during and following sorption.
