Evaluation of extracellular matrix in non tumorous changes and transitional cell tumors of the urinary bladder

In order to investigate the significance of changes in the extracellular matrix of urothelium in different histological forms of chornic cystitis, transitional cell papilloma, and transitional cell carcinoma of the urinary bladder, laminin, type IV collagen, and fibronectin were studied immunohistochemically. The materials used consisted of tumor tissues taken from 27 patients operated for transitional cell tumors of the bladder and multiple biopsies taken from the bladder wall of 68 patients with chronic cystitis. As the result of the observation on Brunn's nests, cystitis cystica and cystitis glandularis with cellular atypia, it was found that there was a loss of pericellular fibronectin and laminin in the earlier stage and later decreased type IV collagen in basement membrane. More severe alterations of extracellular matrix was found in transitional cell carcinoma. On the basis of these findings, we discussed the possibility of the utilization of immunohistochemical determination of these proteins in the extracellular matrix as an informative test to know the outcome of non-tumorous changes of urothelium and the prognosis of transitional cell tumors of the bladder.
