The authors submit their initial experience with selective papillotomy in the direction of the pancreatic duct (EPPT). Indication for the procedure in six patients were: constant pain of pancreatic origin and frequent origin and frequent attacks of pancreatitis and significant postinflammatory changes of the pancreas during ERCP examinations, stenosis of the papilla Vateri manifested by a significantly retarded evacuation of contrast material from the pancreatic duct. Concrement was removed from the pancreatic duct in one patient after papillectomy; marked improvement of the patients' condition occurred in two cases, in one patient the condition improved only, as the subjective complaints persisted, though they were less intense, and in three instances no detectable improvement was recorded. The possible cause of 50% failure is the fact that papillotomy of the duct was performed without subsequent drainage. The operation was not associated with serious complications in any of the patients.
