An autopsy case of intoxication with Disyston granule, organophosphate insecticide

A case of intoxication with Disyston, an organophosphate insecticide is reported. At inspection of the deceased, pinpoint pupils, evacuation of the bowels, and fine foam on the mouth were found. Autopsy revealed congestion and edema of the lungs, fine foam in the trachea, and congestion of the liver, spleen and kidneys. In the gastrointestinal duct, water-like contents had strong garlicky smell and "sand ". Serum cholinesterase value was 0. 1 DELTA-pH. Toxicological examination detected ethylthiometon from the fine foam on the mouth, heart blood (0.65 mu-g/ MI) and stomach contents (117.7 mu-g/ml). Disyston contains 5% of ethylthiometon and 95% of mineral sand. From the weight of "sand", it was suspected that the ingestion volume of ethylthiometon was about 1.5 g.
