Application of personal method for decompression in simultaneously realized colo-rectal and ileorectal or colo-rectal and ileo-cecal anastomosis after removal of locally advanced sigmoid-rectal cancer

Application of personal method for decompression in simultaneously realized colo-rectal and ileorectal or colo-rectal and ileo-cecal anastomosis after removal of locally advanced sigmoid-rectal cancer

Problemi na Onkologiyata 19(0): 46-52

After removal of locally advanced tumours on the transition rectum-sygma and partial resection of the ileum, decompression was realized in two variants: 1. When resection of the middle part of the ileum was made, termino-terminal colo-rectal and ileoileal anastomosis was performed. A wide Foley's catheter was introduced by opening of the coecum into the ileum and the balloon of the catheter was left proximally from the anastomosis. 2. When resection of the distal part of the ileum was made, the coecum and ascending colon were mobilized. The remnant of the ileum together with the coecum were turned at 90-120 degree clockwise. The latter was brought up to the anterior abdominal wall. Through it Foley's catheter was placed, the balloon standing proximally from the ileocoecal anastomosis. In both cases the catheter balloon was expanded to obturate the lumen of the small intestine. The liquid smallintestinal content was discharged through the catheter as in definitive ileostoma. After two weeks the urethral catheter was removed. In the first variant the coecostome spontaneously, closed while in the second variant the ileostoma extraperitoneally closed up to the Bauchin's valve, itself.
