Evaluation of cardiac abnormalities in patients with systemic sclerosis by echocardiography and Doppler

To evaluate the prevalence and characteristics of cardiac involvement and determine the usefulness of echocardiography with Doppler ultrasound in the management of patients with systemic sclerosis, we studied 55 consecutive patients over 4 years. Pericardial effusion was the most frequent abnormality on 2-dimensional echocardiogram which was found in 13 patients (24%), followed by right ventricular dilatation (16%), septal hypertrophy (9%) and septal displacement toward the left ventricle (9%). Tricuspid regurgitation (TR) was the most frequent abnormality observed in 31 (56%) subjects on Doppler echocardiogram. A significant (p lt 0.01) correlation was noted between pulmonary artery systolic pressures estimated from Doppler TR velocities and catheterization data obtained nonsimultaneously in 12 patients. Echocardiography with Doppler examination was useful in the diagnosis and follow-up of 12 patients who developed overt cardiac diseases, including 4 with cor pulmonale, 2 with valvular disease, 2 with suspected myocardial fibrosis and 1 with myocardial infarction. Although an abnormal pattern of left ventricular filling was observed in 14 (25%) patients, its clinical significance could not be determined by our study.
